Scope Of Our Programs
Services and Care
We provide adult day services for functioning impaired adults requiring assistance to safely remain independent in the community.
Participants attend on a scheduled basis, and we provide them with an individualized and coordinated set of services that includes health services, social services, and nutrition services that are directed at maintaining or improving their capabilities of self-care.
We prepare an individual service plan for each participant designed to improve maintaining or delay the decline of their physical, social, and mental functioning. We provide opportunities for participants to seek employment if the need is identified in their Individual Services Plan.
Services and Care
Home health aides are specially trained certified nurse aides who work under the supervision of a registered nurse.
Our Activities Include
A daily exercise program (For participants whose physicians have authorized their participants).
Social activities that provide opportunities for social interaction and friendship formation, such as games designed to create interaction between out participants, group activities and neighborhood walks.
Therapeutic activities, such as arts and crafts projects, memory games, and musical entertainment, designed to provide opportunities to learn new ideas and skills, to rekindle old skills and to continue personal growth event among our most frail or impaired participants.
Population We Serve
We provide services for “functionally impaired adults” required care and supervision who are 65 years old or older. They may have limited ability to perform one or more of the essential major activities or daily living and have physical and/or cognitive impairments.
Participants must be able to transfer with minimal assistance and be able to feed themselves.
